Client Registration Process

All clients must complete the registration process at our agency office before services begin. This process helps us understand each client's unique needs and ensures we provide the most appropriate level of care.

Following the assessment, a personalized care plan is developed based on the client's specific needs. This care plan is maintained in the client’s file and reviewed regularly to ensure the quality and effectiveness of services.

During registration, clients will be required to:

  • Complete the agency application form

  • Submit required documentation

  • Participate in an initial client assessment

Personalized Care Plans

Each client receives a customized care plan tailored to their individual situation. The care plan outlines the services to be provided and the level of support required.

Care plans are:

Caregiver Screening & Requirements

Service Standards

Healing Hearts caregivers provide non-medical assistance only. Our caregivers do not administer medications or provide medical treatment.

Complaints & Concerns

Healing Hearts maintains a formal process for addressing any complaints or concerns raised by clients or their families.
